What to Bring With on Your First Visit to Our Office

PLEASE BRING ALL OF THE FOLLOWING WHEN YOU COME IN

A.

1. Pay stubs for the past sixty (60) days for you and your spouse (if any).
2. All documents that show the amount that someone else contributes to household expenses.
3. Proof of other sources of income such as pensions; interest; support; severance pay; rental income for the past six months.
4. If you are in business, proof of your gross business income and business expenses for the past six months.
   
   B. HOME: (Including Mobile Homes)
5. Copy of deeds to property owned in the last three years, including timeshares and condominiums.
6. Mortgage, home equity loan or line of credit statements.
7. Property tax bill.
8. Statements for utilities; water, sewer; garbage.
9. Homeowners association dues or fees: most recent statement.
10. Homeowners or renters insurance statement, most recent.
    
    C. VEHICLES:
11. Vehicle registrations for each vehicle.
12. Proof of insurance.
13. Vehicle loan statement.
    
    D. FINANCIAL:
14. Credit card statements for past three months, including letters from collection agencies.
15. Bank Statements for each bank, credit union, and investment or stock trading account
16. Checkbook register for the last 6 months.
17. Statement for any IRA, 401K , education individual retirement account, tuition credit program or medical savings plan.
    
    E. DOMESTIC MATTERS:
18. Child or family support orders.
19. Prenuptial or post nuptial agreements.
20. Registration certificate for domestic partnerships
21. Driver's license, MN. ID, or passport.
22. SS Card., a W-2; or tax notice showing your Social Security number.
    
    G. MISCELLANEOUS:
23. Contracts for purchase of vehicle, electronics, jewelry, etc. over time within the past three years.
24. Lawsuits filed against you or by you in the past two years.
25. Any other corporate entities you may own. Get me financials.
26. Get me documentation of any garnishments.
